Medical Home

The mission of the National Center of Medical Home Initiatives is to work in cooperation with federal agencies, particularly the Maternal and Child Health Bureau (MCHB), to ensure that children with special needs have access to a medical home. The National Center provides support to physicians, families, and other medical and non-medical providers who care for children and youth with special needs.

Through the National Center, physicians, parents, administrators, and other health care professionals have access to educational, resource, and advocacy materials, guidelines for care, evaluation tools, and technical assistance.

Medical Home Grants

The Medical Home Grants are to promote access to ongoing comprehensive care through a medical home for all children with special health care needs (CSHCN). This page contains a list of past and present MCHB Medical Home Grantees and their project abstracts. Grant initiatives include:

* Medical Home Implementation through Community-Based, Primary Care Practices

* Subspecialty Capacity-Building

* Early Identification and Intervention for Children with Autism

* Statewide Medical Home Development Grants

Community Access to Child Health (CATCH) Grants

To help pediatricians achieve the goal of increasing the number of medical homes for children with special health care needs, the CATCH program funds projects that investigate ways in which physicians can create medical homes for children with special health care needs within their practices and/or communities.

This program supports pediatricians in planning and implementation of community-based child health initiatives. Six-month grants of up to $10,000 (up to $3,000 for resident grants) are awarded each year on a competitive basis.
